Job Title: Community Relations Coordinator

Job Summary: This position is responsible for the marketing and leasing of the Assisted Living apartments. Work closely with the Manager as a strong team player to make sure the Community is operating at 100% occupancy. This position requires being on call weekends and/or evenings to accommodate prospects and family visits, internal and external marketing events as requested.

Job Title: Resident Assistant

Community: Prairie Hills at Tipton

Job Summary: The Resident Care Partner is a non-professional employee trained to provide personal care and related services to residents in the community according to documented, individualized service and care plans. He/she functions under the Healthcare Coordinator. May be a Certified Nurse’s Aide but this is not required.

Required Skills and Abilities:

  • Obtains vital signs, records and notifies the supervising nurse of deviations per Individual Service Plan (ISP) as per resident’s tasks on POC or EMAR orders.
  • May provide assistance with the following: Personal hygiene (bathing, grooming, hair care, shaving, skin care), Change bed linens and make bed, Toileting and Incontinent Care, Nutritional and fluid intake, Dressing, Ambulation and transfers, Positioning, Medication reminders, Visual checks on residents as assigned, Laundry and housekeeping tasks, Other duties as assigned within scope of practice
  • Follows the resident’s ISP as directed. Documents tasks in POC and timely medication administration in EMAR.
  • Communicates with the appropriate supervisor about changes in the resident’s status and/or the home situation
  • Utilizes standard precautions and infection control procedures
  • Follows written instructions and requests assistance and further instructions with new or unfamiliar situations.
  • Assists the resident in maintaining a safe and clean environment
  • Exhibits a caring and respectful attitude toward the resident and the resident’s property
  • Assist with activities and encourage resident engagement and participation
  • Uphold confidentiality of residents and coworkers, HIPAA
  • Attend Bi weekly in service meetings and mandatory meetings
  • Adheres to all community and company policies
  • Performs any other duties as assigned

Job Title: Registered Nurse

Community: Prairie Hills at Independence, Prairie Hills at Clinton

Job Summary: Work in partnership with the Healthcare to deliver the Red Carpet Experience for Residents and team Members. Responsible for planning developing, organizing, implementing and directing people-centered care programs for all residents. Deliver and direct the company’s brand and program standards that enhances the physical, social, emotional, spiritual, and intellectual needs of residents. Ability to train and lead Resident Assistant to deliver personal care and act in accordance with the five core values and four core principles. Provides coordination and direction of the resident care staff for assisted living and memory care. Familiar with a variety of the field’s concepts, practices, and procedures. Present in-services and act as a leader in all meetings and training sessions. Maintain resident files with all forms required by state law. Relies on extensive experience and judgment to plan and accomplish goals. Performs a variety of tasks. Leads and directs the work of others. A wide degree of creativity, initiative, autonomy and latitude is expected.

Required Skills and Abilities:

  • Work in partnership with the Healthcare Coordinator to lead community culture that acts with compassion and integrity.
  • Coordinate and implement the delivery of resident care.
  • Complete assessments and write service plans for current and prospective residents as required by company policies.
  • Responsible for (EHR) Electronic Health Record.
  • Be responsible for notifying doctor and family of any health concerns with the discretion of the HCC.
  • Be responsible for documenting residents’ health concerns in Nurse’s Notes.
  • Be responsible for having EMAR prepared, accurately, monthly and as needed.
  • Monitor medications for all residents who receive medication management every 30 days.
  • Monitor PCC Clinical Dashboard multiple times per day for task and med documentation.
  • Assure that appropriate medical information is relayed between shifts.
  • Assist to Delegate and document tasks according to the healthcare manual and the Nurse Practice Act.
  • Be responsible for overseeing all medication reminders and administration.
  • Conduct health care related portion of the new hire orientation and onboarding.
  • Train each new and existing employee, thoroughly, in his/her duties and in the Policies and Procedures, as they relate to their position.

Job Title: Assistant Cook

Community: Prairie Hills at Tipton

Job Summary: The Assistant Cook will assist in the operation of the kitchen, dining room, dish room, and related storage spaces. This position will also substitute for the Culinary Service Coordinator when that person is not on site and have the overall responsibility for operating the Culinary Service Department in a quality manner as specified in the Policies and Procedures Manual. Hours may vary from 6:00AM – 2:30 PM or 10:00 AM – 6 PM or as scheduled by the Culinary Service Coordinator and every other weekend.

Job Title: Culinary Service Aide

Job Summary: The Culinary Service Aide will assist in the operation of the kitchen, dining room, dish room, and related storage spaces. Hours may vary from 7:30 AM –2:00 PM and/or 4:00 PM – 7:00 PM and every other weekend as scheduled by the Culinary Service Coordinator.
